ABSTRACT

The authors report two cases of adrenal insufficiency secondary to infiltration of the adrenal glands by Paracoccidioides brasiliensis. The first patient had been treated for a chronic multifocal form of paracoccidiodomycosis 11 years ago. The diagnosis of the mycosis was done simultaneous with that of the adrenal insufficiency in the second patient. In both patients the diagnosis was done by direct visualization of fungus in adrenal biopsies. They were treated with hormonal supplements and itraconazol by 12 and six months, without relapses during the follow-up period.


Os autores apresentam dois casos de insuficiência supra-renal secundária à infiltração das adrenais pelo Paracoccidioides brasiliensis. O primeiro paciente tinha sido tratado de paracoccidioidomicose crônica multifocal 11 anos atrás. No segundo paciente, o diagnóstico da micose foi feito de forma simultânea com o da insuficiência adrenal. Em ambos os pacientes, o diagnóstico foi feito pela visualização direta do fungo nas biopsias adrenais. Eles foram tratados com suplementos hormonais com itraconazol por seis a 12 meses, sem recaídas durante o período de acompanhamento.

ABSTRACT

Histoplasmosis is a fungal infection caused by Histoplasma capsulatum. In the normal individual, both disseminated histoplasmosis and symptomatic adrenal histoplasmosis are rare. Herein, we describe the case of a 50-year-old gentleman residing in western Sudan who presented with a 7-month history of generalized body weakness, easy fatigue, and frequent attacks of vomiting and diarrhea. Physical examination and laboratory investigations confirmed the diagnosis of Addison's disease due to Histoplasma capsulatum var duboisii infection of the adrenal glands. He was treated with intravenous hydrocortisone, followed by oral prednisolone and itraconazole

ABSTRACT

La paracoccidioidomicosis es una micosis profunda, enfermedad granulomatosa subaguda o crónica, que compromete la piel, mucosas, ganglios linfáticos y órganos internos, de evolución maligna, sin tendencia a la curación espontánea. Realizamos una revisión de lo publicado sobre esta patología y reportamos un caso que representa una forma visceral pura, de presentación rara, aunque existen unos pocos casos publicados sobre todo en la literatura peruana y brasilera.


Paracoccidioidomycosis is a deep mycosis, that causes a sub acute or chronic granulomatous disease and involves skin, lymph nodes and internal organs. We report a 54 years old male coming from Paraguay that consulted for nausea, vomiting, asthenia and weight loss, lasting one year. An abdominal CAT scan showed a right adrenal tumor and a left adrenal hyperplasia. A percutaneous biopsy of the adrenal gland under CAT guidance disclosed a paracoccidioidomycosis. The patient was treated with amphotericin B, with good results.

ABSTRACT

We report a case history of an 84-year-old elderly male patient that presented with a clinical picture suggestive of adrenal failure and bilateral adrenal nodules detected by abdominal computed tomography. A fine needle-guided biopsy was inconclusive for achieving a final diagnosis. The patient died due to septic shock and the autopsy disclosed histoplasmosis with extensive bilateral necrosis of the adrenal glands. Although the adrenal involvement in chronic disseminated histoplasmosis has been described, there have been few reports of the infection being associated with adrenal insufficiency.

ABSTRACT

A 40 year old lady presented with the classical clinical features of Addison's disease which on further investigations with an ultrasound abdomen showed a right suprarenal mass. This was subjected to a fine needle aspiration which revealed pus which on culture grew Klebsiella pneumoniae. Patient responded well to steroids and antibiotics. To the best of our knowledge this is the first report of Klebsiella pneumoniae in association with Addison's disease.

ABSTRACT

We report a case of disseminated histoplasmosis in a 60-year-old non-immunocompromised patient who presented to us with fever and hepatosplenomegaly. Sonographic & CT examination of the abdomen showed bilateral adrenal masses. Cytological examination of the aspirated material from the mass showed yeast forms of H. capsulatum.
